The successful candidate will provide expert care to patients, helping them overcome physical, mental, or developmental challenges that limit their ability to perform daily activities. As a Mobile Rehabilitation Therapist, you will work closely with patients and their families to develop personalized treatment plans, addressing specific needs and goals.

Key Responsibilities:
  • Assessment and Evaluation
    • Conduct thorough assessments to understand patients' needs, strengths, limitations, and goals.
    • Utilize standardized tests, interviews, and observations to evaluate physical, cognitive, and emotional abilities.
    • Create comprehensive evaluation reports highlighting areas for improvement and potential interventions.
  • Developing Treatment Plans
    • Design customized treatment plans based on assessment results, focusing on individual patient needs.
    • Establish measurable goals for improvement in self-care, mobility, work performance, and cognitive skills.
    • Recommend tailored therapeutic activities and exercises aligned with the patient's abilities and progress.
  • Providing Therapeutic Interventions
    • Implement individualized therapy programs to enhance motor skills, cognitive function, sensory processing, and social interaction.
    • Engage patients in activities such as exercises, therapeutic games, fine motor skills tasks, and adaptive strategies to promote independence.
    • Teach patients new techniques for completing daily tasks and adapting to assistive devices for mobility.
  • Patient Education and Support
    • Educate patients and their families about the rehabilitation process, including techniques and strategies for enhancing functional independence.
    • Provide guidance on using adaptive equipment and tools, such as splints and mobility aids.
    • Offer emotional support and encouragement throughout the recovery process.
  • Collaboration and Communication
    • Work collaboratively with healthcare professionals, including doctors, physical therapists, speech therapists, and nursing staff, to provide comprehensive care.
    • Participate in care planning meetings and share insights about patient progress and therapy requirements.
    • Regularly communicate with families and caregivers to update them on progress and offer guidance on supporting therapy goals at home.
  • Documentation and Reporting
    • Maintain accurate and detailed records of patient progress, therapy sessions, and any modifications made to treatment plans.
    • Document and report patient progress in compliance with healthcare regulations (e.g., HIPAA).
    • Complete necessary reports and update care teams on patient progress, challenges, and success.
  • Rehabilitation and Recovery
    • Monitor and reassess patient progress regularly to ensure therapy goals are being met.
    • Adjust treatment plans as necessary based on patient feedback and progress.
    • Help patients transition from therapy to independent living, ensuring they are prepared to manage daily tasks outside of a clinical environment.
  • Advocacy and Community Resources
    • Advocate for patients to ensure access to necessary resources and support services.
    • Provide patients and families with information about community programs, support groups, and other resources that may aid in their rehabilitation.
  • Maintaining Continuing Education
    • Stay current with new developments in occupational therapy by participating in workshops, seminars, and continuing education courses.
    • Ensure licensure and certifications remain up-to-date.
