About the Role

CenterWell Home Health is seeking a skilled Occupational Therapist to join our team. As a Home Health Occupational Therapist, you will work with patients in their homes to help them recover from injury or illness, and regain their independence.

Key Responsibilities:

  • Assess and screen patients' daily living and work-related skills, and develop therapeutic retraining programs with measurable objectives.
  • Administer and interpret diagnostic and prognostic tests to assist the physician in evaluating patient function.
  • Confer with the patient's physician and other healthcare team members to develop and revise the Plan of Care Treatment.
  • Provide therapeutic treatment and instruction to patients in accordance with physician orders to improve or restore strength, coordination, range-of-motion, and function.
  • Review and expand retraining programs to align with patient behavioral goals and rehabilitation program guidelines.
  • Consult with vocational team members to develop and implement vocationally oriented plans consistent with patient needs and capabilities.
  • Report patient status and progress to the physician, rehabilitation staff, Clinical Manager, patient, and family members.
  • Participate in care coordination and discharge planning activities, and act as a resource to other healthcare personnel in meeting patient needs.
  • Design community reintegration activities to assist clients in physical reconditioning, and coordinate plans with interdisciplinary team members.
  • Recommend and design special adaptive equipment to improve residence or working environments, or improve participation in the rehabilitation program.
  • Design and train staff and family members to carry out retraining programs, including dressing, feeding, grooming, and hygiene skills.
  • Coordinate and oversee Occupational Therapy Assistant and Home Health Aide performance in implementing occupational therapy services.
  • Accurately and thoroughly document patients' care observations, interventions, and evaluation on the day services are rendered.

Requirements:

  • Degree from an accredited Occupational Therapy Program.
  • A minimum of six months of occupational therapy experience preferred.
  • Home Health experience a plus.
  • Current and unrestricted OT licensure.
  • Current CPR certification.
  • Good organizational and communication skills.
  • Valid driver's license, auto insurance, and reliable transportation.
