Social Work Specialist

We are seeking a highly skilled and compassionate Social Work Master of Social Work to join our team at ChristianaCare in Wilmington, Delaware.
This is an exciting opportunity to work in a dynamic and patient-centered environment, where you will have the chance to make a meaningful impact on the lives of our patients and their families.
Key Responsibilities- Provide expert social work services to patients and their families, addressing psycho-social barriers and supporting discharge needs.
- Collaborate with our Care Management Triad Team Model, working closely with RN Case Managers and Utilization Management specialists to ensure seamless patient care.
- Develop and implement effective care plans, taking into account the unique needs and circumstances of each patient.
- Communicate effectively with patients, families, and healthcare teams to ensure comprehensive and coordinated care.
- Master's degree in Social Work (MSW) or Master of Social Services degree (MSS) from an accredited graduate school.
- Licensed CSW or MSW as required by the state of Delaware.
- Acute Care Social Work experience preferred.
- Maternal-Child position requires experience working with neonates, children, and families.
