About the Role

As a Home Risk Assessment Specialist, you will conduct comprehensive health assessments in the comfort of patients' homes, reviewing and observing their current health, medical history, medication adherence, social environment, and other risks.

This provides unmatched insight into a patient's overall health and well-being, making it easier to capture during routine office visits. You will work collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as needed.

Responsibilities

Your responsibilities will include:

  • Conducting adult and geriatric assessments, medical history, diagnosis, and treatment
  • Providing health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols
  • Collaborating with Primary Care Physicians (PCPs) on patient education and providing follow-up
  • Working with a nationwide network of advanced practice providers to build and maintain relationships
Requirements

To be successful in this role, you will need:

  • A Master's degree or commensurate experience and satisfactory completion of NP licensure
  • Current RN and NP licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority
  • Board certification by the AANP, ANCC, or AACCN in a Matrix-approved specialty
  • A current BLS, ACLS, or CPR certification
  • A minimum of 1 year of experience as a Nurse Practitioner
  • Strong computer skills and familiarity with employee health/medical record software
  • Excellent verbal and written communication skills with patients, clients, and colleagues
  • The ability to travel, a valid state driver's license, and adequate automobile insurance coverage
