Job Title: Registered Nurse
Job Description:
We are seeking a skilled Registered Nurse to join our team at Core Medical-Perm. As a Home Health Care Professional, you will provide high-quality medical care to patients in their homes, promoting independence and well-being.
Key Responsibilities:
- Assess patient needs and develop individualized care plans
- Provide direct patient care, including medication administration and wound care
- Collaborate with healthcare teams to ensure seamless care transitions
- Monitor patient progress and adjust care plans as needed
Requirements:
- Current RN licensure in the state of New Hampshire
- At least 1 year of experience in home health care or a related field
- Strong communication and interpersonal skills
