This opportunity requires travel within Delaware, and we are looking for someone who can work effectively in a fast-paced environment while maintaining excellent communication skills. Key Responsibilities
  • Assess and develop comprehensive care plans for members with complex and chronic healthcare needs
  • Collaborate with healthcare professionals to ensure seamless delivery of care services
  • Travel within Delaware to conduct assessments and provide care coordination services
  • Ensure adherence to regulatory guidelines and quality performance standards
Requirements

To be successful in this role, you will need:

  • A Bachelor's degree in Nursing (BSN) or equivalent
  • 2+ years of experience in case management, discharge planning, or a related field
  • Experience working with geriatric special needs, behavioral health, home health, or similar populations
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a fast-paced environment
Benefits and Salary

We offer a competitive salary range of $80,000 - $110,000 per year, depending on experience, as well as a comprehensive benefits package that includes medical, dental, and vision coverage, 401(k) matching, and paid time off.
