Patient Care Specialist

We are seeking a compassionate and skilled RN to visit patients in their homes, complete assessments, administer medication, and educate patients and their families on home health care strategies. This role involves one-on-one visits, vital sign monitoring, and medication review.
As a Patient Care Specialist with St. Joseph's Home Health, Inc., you will play a critical role in educating patients on proper care and making recommendations for improving their quality of life. You must have a diploma or bachelor's degree in nursing, current RN licensure, CPR certification, and excellent communication skills. Bilingual ability is a plus.
You will work a flexible schedule and require reliable transportation. In return, we offer a competitive salary range of $65,000 - $90,000 per year, depending on experience, as well as health insurance, paid time off, and a sign-on bonus.
