Job Summary

The Physical Therapist Case Manager is responsible for overseeing all aspects of patient care, including supervision of Physical Therapy Assistants and Home Health Aides. This role promotes patients' potential during their home health episode and collaborates with the physician, clinical team, and patient in preparation for discharge.

Key Responsibilities
  • Supervise and manage all aspects of patient care, including PTA and HHA supervision
  • Coordinate the distribution of visits among all disciplines
  • Initiate referrals to ancillary services
  • Facilitate interdisciplinary communication
  • Lead weekly case conferences and contribute to care plan development
  • Establish social action plans and realistic goals for patients
  • Coordinate discharge planning with patients, clinical team, and agency
  • Provide individual and group support for patients and families
  • Complete necessary documentation, including evaluations, visit notes, and discharges
Requirements
  • Physical Therapist licensed per state requirements
  • Skills in communication, documentation, assessment, and social work methods and techniques
  • Ability to read, write, and speak English
