Job Summary

We are seeking a highly skilled Care Coordinator Liaison to join our team. In this role, you will work under the direction of Registered Nurses (RNs) and/or Social Workers to coordinate post-acute services for patients. Your responsibilities will include arranging post-discharge services, creating and following up on electronic referrals, reviewing patient choice letters, providing follow-up communication to Medicare patients, and completing various administrative tasks.

Main Responsibilities
  • Makes referrals for post-acute services under the direction of RN Case Managers or Social Work staff, utilizing the electronic Tenet Case Management system.
  • Provides patients and families with choices of post-acute providers per Tenet policy.
  • Responds to post-acute providers timely, and completes referrals per Tenet policy.
  • Documents and communicates all elements of the post-acute referral to the RN Case Manager or SW, and the healthcare team, patient/family, and post-acute providers.
  • Completes tasks as assigned by RN or LVN Case Manager and/or SW staff.
  • Makes copies, sends faxes, and completes phone calls to arrange post-acute services and ensure that appropriate hospital information is communicated to post-acute providers.
  • Documents all referrals and tasks in the Tenet Case Management system per Tenet policy.
  • Provides patients and healthcare teams with information regarding resources and benefits available to patients along with the economic impact of care options.
Requirements

To be successful in this role, you must have:

  • A High School Diploma or Equivalent.
  • At least 2 years of experience in a clerical or healthcare field.
  • Preferred: Acute hospital experience, Paramedic, EMT, or Certified Nursing Assistant certification.
Compensation and Benefits

The estimated salary for this position is $60,000 - $80,000 per year, depending on experience. Additionally, we offer a comprehensive benefits package, including medical, dental, and vision insurance, retirement plan, and paid time off.

Tenet complies with federal, state, and local laws regarding mandatory vaccination of its workforce. If you are offered this position and must be vaccinated under any applicable law, you will be required to show proof of full vaccination or obtain an approval of a religious or medical exemption prior to your start date.
