Caregiving Clinician

We offer services to patients in San Francisco, San Mateo, Santa Clara, Santa Cruz, Alameda, Contra Costa, Solano, and Napa Counties, with plans to expand into Yolo and Sacramento Counties soon.

As a Caregiving Clinician, you will be responsible for conducting initial assessments of patients to determine their home care needs. This includes performing start-of-care admissions, follow-up visits, and evaluations as needed within designated territories. You will also make home visits to patients, perform assigned duties such as reconciliation of medication, wound care, treatments, and procedures, and collaborate with physicians and agency staff to deliver the best possible care.

• Conduct Start of Care (SOC) Admissions
• Follow up visits and evaluation visits as needed in designated territory
• Make home visits to patients in designated territory
• Perform assigned duties including reconciliation of medication, wound care, treatments, and procedures
• Monitor clients' conditions and report changes to Clinical Team
• Collaborate with physicians and agency staff to deliver the best possible care

• Current and unrestricted CA RN license
• Current BLS/CPR Certification
• Valid CA Driver's License
• Experience conducting assessments and SOC is highly desired
• High level of clinical knowledge in home health care is highly preferred
• Experience working with high acuity patients is a plus
• Self-directed and empathetic individual with outstanding interpersonal skills

Estimated Salary: $85,000 - $110,000 per year, based on experience and location. Benefits include competitive compensation, achievable incentive bonus program, generous PTO, medical, dental, life insurance, and more.
