Job Summary:

We are seeking a skilled Healthcare Coordinator to join our team at Centene. As a key member of our care management team, you will play a critical role in developing and implementing personalized care plans for our members.

About the Role:

This is a field-based position that requires travel throughout the state of Delaware. You will be responsible for assessing the needs of our members and developing effective care plans to address their mental and behavioral health needs.

Key Responsibilities:

  • Evaluate member needs through phone or in-home visits and recommend care plans to ensure optimal outcomes
  • Develop ongoing care plans for members with high acuity and identify necessary providers, specialists, and community resources
  • Coordinate between members, caregivers, and healthcare providers to ensure access to needed services
  • Monitor member status and outcomes and provide recommendations for changes in treatment
  • Facilitate care coordination and collaborate with providers to ensure timely access to care

Requirements:

To be successful in this role, you must have a Master's degree in Behavioral Health or Social Work or a Degree from an Accredited School of Nursing and 2-4 years of related experience. A Licensed Behavioral Health Professional or RN based on state contract requirements is also required.

Compensation:

The estimated salary for this position is $54,000 - $97,100 per year, depending on your skills, experience, education, and other job-related factors.
