Job Summary

Providence Health & Services is seeking a Clinical Pharmacist to join our team in Valdez, Alaska. As a Clinical Pharmacist, you will be responsible for providing high-quality, individualized, cost-effective pharmaceutical care and integrated pharmacy operational services in collaboration with the healthcare team to optimize patient's medication therapy outcomes and quality of life.

Responsibilities
  • Provide pharmaceutical care to patients in a hospital setting
  • Collaborate with the healthcare team to optimize patient's medication therapy outcomes and quality of life
  • Develop and implement individualized medication therapy plans
  • Monitor and evaluate patient's response to medication therapy
  • Provide education to patients and healthcare team members on medication therapy

Requirements
  • Bachelor's Degree in Pharmacy or PharmD
  • Alaska Pharmacist License
  • Basic Life Support-Provider (BLS)

Preferred Qualifications
  • Acute care hospital pharmacy experience
  • Completion of ASHP-accredited PGY1 residency
