About the Position:

As a Geriatric Care Specialist, you'll be accountable for caring for patients, maintaining accurate and current patient records, and scheduling initial, urgent, and follow-up appointments as required. You'll work closely with our physicians, social workers, paramedics, and care team coordinators to deliver concierge care to every patient.

Key Responsibilities:

  • Practice our mission: To help patients lead healthier, meaningful lives by delivering the most Complete Care.
  • Partner with other clinicians in the Care Community to manage the health and wellbeing of patients living in their homes.
  • Responsible for engaging attributed patients into our program.
  • Provide primary care to patients in the home.
  • Facilitate staff, patient, and family decision making by providing educational resources.
  • Perform initial patient screenings/physical examinations, create appropriate assessment/plans, address urgent care needs, collaborate with community teams, order, interpret, and analyze diagnostic tests, prescribe and manage medications, and update patient records.
  • Serve as a resource for patients in delivering ongoing care information, counseling, and guidance for critical conditions.
  • Refer to internal/external resources as needed.
  • Collaborate with the interdisciplinary team and participate in regularly scheduled team meetings.
  • Maintain required licensure and certification.
  • Deliver palliative care and end-of-life care to patients and families.

Salary: $85,000 - $110,000 per year
