Healthcare Manager RN for WellMed at Cleburne

As a RN Healthcare Manager for WellMed in Cleburne, you'll play a key role in reducing ER utilization, hospital admissions, and readmissions. You'll work collaboratively with the multidisciplinary care team to support the transition of high-risk patients from the inpatient to home setting and provide longitudinal care for patients with complex, chronic conditions. This role requires a strong understanding of chronic diseases, especially COPD/asthma, diabetes, CHF, and IHD. You'll also assist in developing patient engagement by acting as an educator, resource, and advocate for patients and their families. This position functions as an independent role within the clinic setting. Key responsibilities include:
* Utilizing population stratification tools to identify high-risk patients
* Completing transition of care processes, including prompt provider follow-up, medication reconciliation post-discharge, and contingency planning
* Performing longitudinal and/or proactive outreach assessments
* Creating referrals to appropriate agencies and community resources
* Establishing plans of care for patients and coordinating advance care planning
Required qualifications include a Bachelor's degree in Nursing or Associate's degree in Nursing with 3+ years of ambulatory experience in a physician's office or clinical setting. You'll also need to be a Registered Nurse with an active and unrestricted license to practice in the state of employment and have current BLS certification. Additionally, you'll need to be proficient in computer skills to work efficiently with electronic medical records. This position requires Tuberculosis screening as well as proof of immunity to Measles, Mumps, Rubella, Varicella, Tetanus, Diphtheria, and Pertussis through lab confirmation of immunity, documented evidence of vaccination, or a doctor's diagnosis of disease. Preferred qualifications include acute care experience in hospital and home health settings, experience related to patient education and/or motivational interviewing skills and self-management goal setting, and fluency in written and verbal skills in English and Spanish.
