Job Description:Fieldstone at Chester Springs, the newest senior living community in the area, is seeking compassionate individuals who are passionate about enhancing the lives of seniors. This is a great opportunity to be part of the team that welcomes the community's first residents.
As a Nurse Shift Supervisor, you will be responsible for providing overall leadership and management of the health and well-being of the residents within the community.
Responsibilities:- Maintain effective communication with healthcare providers regarding recent changes or requests for orders.
- Ensure the community adheres to all federal, state, and local laws and regulations related to clinical care.
- Collect and report occurrences, and assist in developing/implementing improvement plans as necessary.
- Collaborate with the Administrator and other team members to maintain resident and team member health and safety.
- Partner with the pharmacy consultant to provide optimal pharmaceutical services to residents.
- Oversee infection control practices in the community.
- Ensure the safe and efficient distribution/administration of medications, injections, nebulizers, and moderate wound care per state regulations.
- Identify ongoing needs and services of residents through the evaluation/Service Plan process, including documentation for residents with a change in healthcare status.
- Manage the completion of assigned evaluations.
- Collaborate with other department coordinators to identify, review, and discuss potential changes in service/medication levels and update the residents' service plan as indicated by state regulations.
- Maintain communication with residents' family and/or responsible party regarding changes in care or health concerns.
- Perform other related duties as assigned by management.
- Associate degree (AA) or equivalent from a two-year college or technical school, or six months to one-year related experience and/or training, or equivalent combination of education and experience.
- Maintains current state license as a Licensed Professional Nurse (LPN) or a Professional Registered Nurse (RN) per state regulations.
- CPR and First Aid Certified
- Experience in assisted living, home health, or long-term care industries preferred.
- Must demonstrate competence in assessment skills, injections/medication administration, follow-up, and triage.
