Cardiologist - Value-Based Care Specialist

The Cardiologist plays a vital role in our team-based care model, partnering with primary care physicians to deliver high-quality care to our patients.
Our care team consists of physicians, advanced practice professionals, medical assistants, and behavioral health support, working together to provide a comprehensive and coordinated approach to patient care.
As a Cardiologist, you will be responsible for evaluating and treating patients, coordinating patient services, and acting as an active participant and key source of medical expertise with the care team.
You will also have the opportunity to work with a quality-based coder to optimize comprehensive documentation and meet with primary care physicians and market medical director to discuss quality of care, review outcome data, and medical records issues.

Key Responsibilities:
- Evaluate and treat patients in accordance with standards of care
- Assist in the coordination of patient services, including specialty referrals and cardiology subspecialists
- Act as an active participant and key source of medical expertise with the care team
- Partner with primary care physicians and nurse practitioners in a tone of cooperation in practice
- Complete all medical record documentation daily, working with a quality-based coder to optimize comprehensive documentation
- Meet with primary care physicians and market medical director to discuss quality of care, review outcome data, and medical records issues
Requirements:
- Graduate of accredited MD or DO program of accredited university
- Current and unrestricted medical license or willing to obtain a medical license in state of practice
- Board Certification Cardiology
- Active and unrestricted DEA license
- Medicare Provider Number
- Medicaid Provider Number
- Demonstrate a high level of skill with interpersonal relationships and communications with colleagues/patients
- Fully engaged in the concept of team-based care model
- Willingness and ability to learn/adapt to practice in a value-based care setting
- Superior patient/customer service
Preferred Qualifications:
- Knowledge of HEDIS/STARS
- Knowledge of CAHPS and HOS
- Experience working in medically underserved/culturally diverse communities
- Experience working with an electronic health record
- Experience working in Medicare Advantage Global Risk Models
