As a Healthcare Navigator, you will work closely with patients and caregivers to provide comprehensive health coaching, coordination, and education. Duties and Responsibilities:

  • Provide health coaching and education to patients and caregivers
  • Collaborate with healthcare providers to develop personalized care plans
  • Conduct assessments and evaluations to monitor progress and outcomes
  • Document interactions and results using electronic platforms
  • Maintain confidentiality and adhere to HIPAA regulations

Requirements:

To succeed in this role, you will need:

  • A graduate degree in nursing or a related field (RN, LPCC, LISW)
  • Current state licensure as a Registered Nurse (RN), Licensed Professional Clinical Counselor (LPCC) or Licensed Independent Social Worker (LISW)
  • At least three years of clinical experience
  • Knowledge of chronic condition programs and federal/state regulations regarding HIPAA

Benefits:

Cleveland Clinic offers a competitive salary range of $65,000 - $90,000 per year, depending on experience. Additionally, our comprehensive benefits package includes medical, dental, and vision insurance, 401(k) matching, paid time off, and more.
