We are seeking an experienced Geriatric Physician to join our team at AgeRight Clinical Services. In this role, you will be responsible for providing primary care and post-hospital medical care to facility residents, collaborating with facility staff and Advanced Practice Providers, and participating in Quality Improvement initiatives.

Responsibilities
  • Primary Care and Post-Hospital Care: Plan, direct, and provide primary care and post-hospital medical care to facility residents.
  • Collaboration and Coordination: Collaborate and coordinate patient care with facility staff and Advanced Practice Providers.
  • Quality Improvement: Participate in Quality Improvement initiatives to improve patient outcomes and reduce hospital readmissions.
  • Documentation: Timely completion of documentation.

Requirements
  • Current Medical License: Current, unrestricted Oregon Medical License.
  • Board Certification: Board certified in Geriatrics, Family Medicine, or Internal Medicine.
  • Active DEA License: Active DEA license.
