We are seeking a dedicated Child and Family Services Specialist to join our team at St. Catherine's Center for Children.

Job Overview

The Child and Family Services Specialist will play a vital role in developing cohesive Child and Family Teams to provide services and support to members and their families. This includes assisting in creating and carrying out integrated and comprehensive Wraparound Plans tailored to individualized needs and strengths.

Responsibilities
  • Maintain a caseload of 10-12 enrolled members and their families, ensuring timely and effective support.
  • Develop Care Teams for enrolled members, utilizing their unique skills and expertise.
  • Develop individualized Crisis and Safety plans, prioritizing the well-being and safety of all involved.
  • Facilitate Child and Family Team Meetings (CFTM), fostering open communication and collaboration among team members.
  • Connect members and families to essential resources, promoting independence and self-sufficiency.
  • Teach and support members and families in learning necessary skills to develop their own plans, access resources, and maintain independence.
  • Understand and implement the 10 Principles of High-Fidelity Wraparound, ensuring best practices in care coordination.
  • Ensure all required billable health home core services and face-to-face contacts are met monthly for all enrolled members.
  • Document essential components of the Wraparound planning process in electronic health records, maintaining accurate and up-to-date information.
  • Comply with timelines for assessment completion, Plan of Care development, and scheduling of initial and ongoing interdisciplinary team meetings.
  • Conduct comprehensive discharge planning with members exiting the program, ensuring a smooth transition to next steps.
  • Participate in monthly Quality Assurance meetings, contributing to continuous improvement and quality enhancement.
Requirements
  • Bachelor's degree in psychology, social work, counseling, sociology, or other human service-related field.
  • Minimum 2 years' experience in child welfare, mental health, substance abuse, and/or physical health.
  • Ability to remain composed under crisis conditions, demonstrating emotional stability and composure.
  • Competence to pass the test to become a certified CANS assessor, ensuring proficiency in assessment tools.
  • Proficient in Microsoft Office Suite, Google Business Suite, and other data processing systems, adhering to secure internet practices.
  • Clean and valid NYS Driver's License, with reliable vehicle for transportation.
  • A compassionate, warm, and caring mindset, capable of exhibiting empathy for those in crisis.
  • Exercise ethical, independent judgment and discretion, handling sensitive data with strict confidentiality.
  • Ability to pass required background checks, including DOH fingerprinting for a criminal history record check.
