Hospice Team Manager

The Team Manager is the leader of the Patient Care Team, responsible for supervising, evaluating, and coordinating the team's activities. They ensure continuity of care from admission to discharge or transfer to bereavement, serve as patient advocate, and coordinate with other healthcare providers.
Key Responsibilities:- Direct patient care services, including volunteer and bereavement services
- Review patient admissions, assessments, and history to ensure eligibility and appropriateness criteria are met
- Collaborate with team physicians and attending physicians on palliative care measures
- Ensure complete and accurate data and documentation for recertification decisions
- Monitor team services, including staffing, medications, and supplies, to ensure they are appropriate and responsive to patient needs
- Manage team schedules and coordinate services 24/7
- Facilitate team meetings and ensure interdisciplinary input into the plan of care
- Participate in on-call rotation and staff supervision and management
Requirements:
- 1 year RN experience in hospice care
- Strong leadership and communication skills
- Ability to work in a fast-paced environment
