We are seeking a highly skilled Clinical Care Coordinator to join our team at Brickyard Healthcare. As an MDS Coordinator, you will play a crucial role in ensuring that our patients receive the care they need and that we have the necessary resources to provide it.

About the Role:
  • This is a full-time position offering a competitive salary range of $ $65,000 - $85,000 per year, depending on experience.
Key Responsibilities:

As a Clinical Care Coordinator, your primary responsibility will be to complete the Resident Assessment Instrument per federal and state regulations and company policy and procedures. You will also act as an in-house case manager by considering all aspects of the resident's care and coordinating services with physicians, families, third party payers and facility staff.

You will be responsible for ensuring that our residents receive high-quality care and that their needs are met in a timely and efficient manner. This may involve communicating with healthcare professionals, families, and other stakeholders to ensure that our residents receive the best possible care.

In addition to these responsibilities, you will also be required to maintain accurate and up-to-date records of resident care and treatment plans. You will work closely with our interdisciplinary team to ensure that our residents receive comprehensive and coordinated care.

Requirements:
  • A Licensed Practical Nurse or Registered Nurse with a current, active license in the state of practice.
  • A minimum of two (2) years of clinical experience in a health care setting.
  • A minimum of one (1) year of experience in a long term care setting.
  • Prior experience as an MDS coordinator with demonstrated expertise.
  • The ability to maintain regular attendance and work effectively in a fast-paced environment.
