Primary Care Provider Position in Connecticut

Job Overview
We are seeking a qualified Primary Care Nurse Practitioner or Physician Assistant to join our team at Healthsphere LLC in East Hartford, Somers, Connecticut.
The ideal candidate will have experience in internal medicine, family medicine, urgent care, or emergency care settings and be comfortable treating a wide range of acute conditions, performing minor procedures, and collaborating with clinical staff.
Key Responsibilities:
- Deliver quality, patient-centered care to patients aged 12 and up in an outpatient setting
- Evaluate, diagnose, and treat a wide variety of acute illnesses and injuries
- Perform minor procedures such as laceration repairs, splinting, incision, and drainage of abscesses
- Order and interpret diagnostic tests (labs, x-rays, etc.)
- Prescribe medications and develop treatment plans for patients
- Provide patient education regarding diagnosis, treatment, and follow-up care
- Conduct thorough medical histories, physical examinations, and diagnostic assessments
- Develop and implement individualized treatment plans, including medication management and lifestyle counseling
- Document patient visits and treatments using the electronic medical records (EMR) system
- Work collaboratively with physicians, medical assistants, and administrative team members
- Provide patient education on chronic disease management, preventive care, and health promotion
Skills and Qualifications:
- Commitment to delivering high-quality patient care efficiently and compassionately
- Ability to provide culturally competent care to a diverse patient population with complex needs
- Proven success in communicating effectively with patients, peers, and other stakeholders
- Demonstrated ability to maintain timely, thorough, and accurate written clinical documentation
- Capacity to make evidence-based clinical decisions and comply with department and organizational quality measures
- Comfortable using electronic health records and other computer systems
- Resourceful problem-solving attitude and a strong sense of accountability
Qualifications:
- NP or PA: Board-certified in Family Medicine, Emergency Medicine, or Internal Medicine
- NP: Active NP license with certification in Family Medicine or Emergency Medicine
- PA: Active PA license with certification and experience in urgent care or emergency medicine
- Current and active Medical License in Connecticut
- Current DEA license (for prescribing controlled substances)
- BLS and ACLS certification required; PALS certification preferred
Estimated Salary: $120,000 - $150,000 per year, commensurate with experience
