We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Greenbriar at the Altamont. As a key member of our healthcare staff, you will be responsible for providing direct care to residents in accordance with their care plans, policies, and procedures.

Key Responsibilities

The Licensed Practical Nurse (LPN) will be responsible for:

  • Providing high-quality patient care, adhering to nursing standards, and collaborating with the interdisciplinary team to ensure resident well-being
  • Assessing residents' physical and emotional status, identifying areas for improvement and developing strategies to address these needs
  • Documenting resident information accurately and thoroughly, maintaining confidentiality and adhering to HIPAA guidelines
  • Communicating effectively with residents, families, and healthcare teams to ensure seamless care coordination
