Job Description: As a Healthcare Referral Specialist at ChenMed, you will play a vital role in coordinating patient care by authorizing, scheduling, and ensuring completion of patient visits with specialty care providers. This includes working with insurance representatives, arranging transportation, communicating with physicians and other medical personnel, and facilitating seamless transitions between primary care, specialists, hospitals, and other healthcare settings.
Responsibilities:
- Serve as primary point of contact for incoming and outgoing patient referrals, triaging referrals and gathering necessary information for timely processing and assignment to appropriate providers.
- Facilitate communication, collaboration, and coordination of care, coordinating appointments, referrals, transitions of care, and ensuring continuity of care.
- Schedule patients utilizing coordinated provider lists (CPL), make all necessary arrangements related to the appointment, notify patients of appointment information, and ensure timely follow-up.
- Use web-based insurance platforms to generate referral authorizations and effectively communicate the needs or outstanding items to patients.
- Follow up on referrals through completed appointments, obtaining documentation and uploading into the organization's medical record system for physician review prior to PCP follow-up appointments.
- Ensure any missed external appointments are rescheduled and communicated to the PCP, addressing referral-related phone calls from patients, providers, etc., and completing phone messages in a timely manner.
Requirements:
- High School diploma or equivalent required.
- A minimum of 1 year of referral experience in a healthcare setting required.
- Experience with web-based insurance sites and obtaining referrals/authorizations for multiple payors preferred.
- Proficient in Microsoft Office Suite products, including Word, Excel, PowerPoint, and Outlook.
- Spoken and written fluency in English; bilingual preferred.
