Hospice Care Coordinator

We are seeking an experienced and compassionate Hospice Registered Nurse (RN) to join our dedicated team as a Case Manager. This role involves coordinating and directing nursing services for patients in need of hospice care, utilizing advanced assessment, teaching, and decision-making skills.
Responsibilities:- We will review patient records and establish care plans based on best practices.
- You will organize your schedule to minimize travel time between visits.
- You will obtain MD orders and implement care plans accurately.
- You will perform comprehensive patient assessments and adjust care plans as needed.
- You will order supplies and durable medical equipment according to hospice criteria.
- You will customize care plans for spiritual, psychosocial, emotional, and physical needs.
- You will evaluate care plan effectiveness and adjust interventions collaboratively.
- You will supervise Licensed Nursing Assistants (LNAs) in compliance with regulations.
- You will document data and interventions accurately in patient charts.
- You will complete admissions and recertifications accurately and timely.
- You will conduct HIS outcome assessments precisely.
- You will participate in Quality Assurance Performance Improvement (QAPI) projects and adhere to clinical policies.
- You will attend required in-services and state trainings, and complete competencies.
- You will communicate effectively with the team and healthcare professionals.
- You will utilize case management concepts in practice.
- You will involve patients and families in the plan of care and IDG meetings.
- You must have current licensure/registration in Nursing in the State of New Hampshire or a Compact State.
- You must have graduation from an approved nursing curriculum by the state and designated nursing governing board.
- Prior experience in med/oncology is preferred, with a minimum of two years of experience.
- Prior experience in hospice is preferred, with a minimum of one year of experience.
- You must be able to continue education to meet state requirements.
- You must be comfortable with technology and able to work independently.
- You must possess strong communication, problem-solving, and decision-making skills.
- You must be able to tolerate prolonged sitting, occasional inclement weather driving, and use a personal cell phone for job-related tasks.
