Key Responsibilities:
  • Conduct comprehensive assessments of patients' health and psychosocial needs.
  • Develop and implement individualized care plans that address patients' goals, expectations, and preferences.
  • Collaborate with the interdisciplinary team, patients, and families to ensure timely and informed decision-making.
  • Provide education and support to patients and families to promote self-determination and empowerment.
  • Ensure appropriate use of healthcare services and allocation of resources to improve quality of care and maintain a balance with cost-effective care.
  • Develop and implement discharge plans that meet patients' needs and financial requirements.
Requirements:
  • Bachelor's degree in a related field (Master's degree preferred).
  • 3-5 years of experience in case management, utilization review, or discharge planning in an acute, rehabilitation, skilled nursing, or outpatient setting.
  • Current, unrestricted license to practice in Massachusetts in a health or human services discipline.
  • Knowledge of community services and healthcare systems.
  • Computer proficiency and ability to learn new software.
Preferred Qualifications:
  • Certification in Case Management.
  • Highly developed communications and interpersonal skills.
  • Ability to work independently and as a member of a team.
