The Role:

We are seeking an experienced Nurse Practitioner to join our team as a House Calls Healthcare Practitioner. In this role, you will provide compassionate, evidence-based medical care to seniors and patients with complex needs in their homes or community settings.

Responsibilities:

  • Conduct thorough assessments and develop personalized care plans for patients
  • Perform routine check-ups, administer medications, and order diagnostic tests as needed
  • Collaborate with healthcare professionals to ensure seamless care coordination and patient outcomes
  • Maintain accurate and up-to-date patient records

Requirements:

  • A valid state NP license and current National Board Certification
  • An active DEA license and Master's degree in Nursing (MSN)
  • At least two years of experience as a Nurse Practitioner, preferably in home healthcare or senior care
  • Excellent communication and interpersonal skills, with the ability to work independently and as part of a team
  • Familiarity with electronic medical records and data management systems

Compensation and Benefits:

This is a full-time position with a competitive salary range of $100,000 to $120,000 per year, based on performance and experience. You will also receive a comprehensive benefits package, including medical, dental, and vision coverage, 401(k) retirement plan, and paid time off.

About the Location:

The successful candidate will be based in Norfolk, VA, with regular visits to Virginia Beach, VA. This is a great opportunity to join a dynamic team and make a real difference in the lives of our patients.
