RN Case Manager

As a skilled RN Case Manager with CenterWell Home Health, you will provide admission, case management, and follow-up skilled nursing visits for patients in the comfort of their own homes. Your primary responsibility will be to administer on-going care and case management for each patient, providing necessary follow-up as directed by the Clinical Manager.
You will work closely with physicians to develop the initial plan of treatment based on their orders and the patient's initial assessment. Your hands-on care, management, and evaluation of the care plan, as well as teaching the patient in accordance with physician orders, under Clinical Manager's supervision, are all key aspects of this role.
Revision of the plan in consultation with the physician based on ongoing assessments and as required by policy/regulation is also a critical part of this position. You will coordinate appropriate care, encompassing various healthcare personnel such as Physical Therapists, Occupational Therapists, Home Health Aides, and external providers.
Reporting patient care/condition/progress to the patient's physician and Clinical Manager on a continuous basis is essential in this role. You will implement the patient care plan in conjunction with the patient and family to assist them in achieving optimal resolution of needs/problems.
Additionally, you will coordinate/oversee/supervise the work of Home Health Aides, Certified Home Health Aides, and Personal Care Workers and provide written personal care instructions/care plan that reflect the current plan of care.
Monitoring the appropriate completion of documentation by home health aides/personal care workers as part of the supervisory/leadership responsibility is also a key aspect of this role. You will discharge patients after consultation with the physician and Clinical Manager, preparing and completing needed clinical documentation.
Your responsibilities will also include preparing appropriate medical documentation on all patients, including any case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes.
Other duties include preparing visit/shift reports, updates/summarizes patient records, and conferring with other health care disciplines in providing optimum patient care.
Required Experience/Skills:
A Diploma, Associate, or Bachelor Degree in Nursing is required for this role. A minimum of one year of nursing experience is preferred, with strong med surg, ICU, ER, acute experience. Home Health experience is a plus.
Current and unrestricted Registered Nurse licensure is necessary, as is Current CPR certification. Strong organizational and communication skills are also essential for success in this role. A valid driver's license, auto insurance, and reliable transportation are required.
