We are seeking a compassionate and skilled End-of-Life Care Specialist to join our team at Corewell Health in Grand Rapids, MI. As a member of our interdisciplinary team, you will play a vital role in providing high-quality end-of-life care to patients and their families.
Job SummaryThe End-of-Life Care Specialist will be responsible for assessing, planning, implementing, coordinating, monitoring, and evaluating the strategies and services required to meet a patient's end-of-life care needs. This includes developing outcome-based plans of care, utilizing the nursing process, relationship-centered care principles, and a holistic philosophy to ensure the changing needs of patients and their families are met.
Responsibilities- Utilize clinical judgment and expertise to coordinate care management and behavioral health interventions.
- Develop strategies to ensure cost-effective, quality care outcomes for patients with complex care needs.
- Identify options for levels of care for individuals and populations of patients, focusing on strategies that promote optimal health.
- Assess, plan, monitor, and provide therapeutic interventions as appropriate.
- Collaborate with patients and their families to facilitate understanding of diagnosis and provide therapeutic interventions to support necessary lifestyle and behavior changes.
- Partner with patients, families, and other healthcare providers to ensure understanding the impact the disease or illness has on the patient's mental and emotional health.
- Respond to crisis situations and engage with patients and families to access the appropriate level of care across the continuum.
To be successful in this role, you must have:
- A Master's Degree in Social Work (MSW).
- MSW Social Work License in the State of Michigan.
- A valid State of Michigan Driver's License.
