Salary: $65,000-$85,000 per year
Job Description
As a Hospice Nurse Coordinator - Weekend RN, you will play a vital role in assessing and identifying patient/family needs, coordinating the Plan of Care, and providing palliative and supportive care to patients and their families.
You will work closely with the Interdisciplinary Group (IDG) to develop an individualized Plan of Care that accurately reflects the patient's evolving needs. Your expertise will be essential in educating patients, families, caregivers, and other health professionals about disease processes, decline, palliative interventions, care giving, dying processes, and safety practices.
Responsibilities
- Assess and identify patient/family needs
- Coordinate the Plan of Care
- Provide palliative and supportive care
- Work with the IDG to evaluate hospice referrals/admissions
- Participate in patient deaths; prepare body for transport
- Provide bereavement interventions to families and caregivers
- Communicate effectively with physicians, staff members, patients, families, and supervisors
- Supervise patient care given by other nursing team members
Required Skills and Qualifications
- Current license as RN in the state where the employee will be working
- Minimum one (1) year of in-patient nursing or hospice experience for external candidates
- Previous experience working with an EMR/EHR system
- Mobile Driver - Valid driver's license and automobile insurance per Company policy
- Ability to use equipment with visual and auditory mechanisms
- Ability to effectively communicate in English (verbal and written)

Must be able to perform the physical demands of the job, which include standing, sitting, reaching, lifting, bending, and balancing. Must have active BLS certification from the American Heart Association or Red Cross. A valid driver's license and automobile insurance are required. Consents to drug and/or alcohol testing after a conditional offer of employment are made, as well as ongoing compliance with the Drug-Free Workplace Policy.
