Job Summary

Elevance Health is seeking a skilled Palliative Nurse Practitioner to join our team as a Field Advanced Practice Provider. This role will involve collaborating with our physicians, patients, and families to develop comprehensive plans of care in the comfort of the patient's home.

Key Responsibilities
  • Develop and implement clinical plans of care for adult patients facing chronic and complex conditions.
  • Provide medical support and education to patients and families, including medication usage, side effects, and illness progression.
  • Collaborate with our inter-disciplinary team to coordinate optimal care and resources for patients and their families.
  • Maintain accurate patient medical records and documentation.
  • Participate in continuing education and professional development to stay up-to-date on best practices in palliative care.
Requirements
  • Current unrestricted Nurse Practitioner (NP) license or Physician Assistant (PA) license in applicable state(s).
  • DEA registration or eligibility required.
  • CPR certification required.
Preferred Qualifications
  • 2 years of experience in home-based services, complex care cases, and hospice and/or palliative care.
  • Bedside hospital, oncology, emergency room (ER) or intensive care unit (ICU) experience highly preferred.
  • Experience working with Electronic Medical Records (EMR) preferred.
  • Bilingual in English/Spanish preferred.
