Job Summary

We are seeking a highly skilled and compassionate Blended Case Manager to join our team at the Community Guidance Center. As a Blended Case Manager, you will be responsible for providing outreach services to consumers, managing their caseload, and implementing service plans to promote growth and change.

Key Responsibilities
  • Provide outreach services to consumers, including home visits, community meetings, and office visits.
  • Manage a caseload of 30 consumers, including assessment, service planning, and monitoring of service delivery.
  • Implement service plans, including writing and implementing goals, linking consumers with services, and monitoring progress.
  • Provide crisis intervention and prevention services, including 24/7 on-call services.
  • Collaborate with other staff members and treatment teams to ensure comprehensive care.
Requirements
  • Bachelor's degree in a related field, or high school diploma with 5 years of related experience.
  • Act 33 & 34 clearances and FBI clearance.
  • Computer skills and ability to work at a desk for intermittent periods.
  • Good public speaking skills and ability to work with diverse populations.
  • Ability to work in a fast-paced environment and manage multiple priorities.
