Job Summary
We are looking for a seasoned Occupational Therapist to provide high-quality care to patients in their homes. As a Senior Occupational Therapist - Home Health Case Manager, you will be responsible for assessing patient needs, developing and implementing personalized care plans, and coordinating with interdisciplinary teams to ensure optimal health outcomes.
Key Responsibilities
- Conduct thorough assessments of patients' physical and cognitive abilities to identify areas for improvement
- Develop and implement tailored care plans to promote independence, mobility, and overall well-being
- Collaborate with healthcare professionals to ensure seamless care coordination and communication
- Provide education and training to patients and their families on proper self-care techniques and safety measures
Requirements
To succeed in this role, you will need:
A graduate degree in Occupational Therapy from an accredited programCurrent licensure as an Occupational Therapist in the state of IllinoisStrong assessment and treatment planning skills, with experience working with diverse patient populationsExcellent communication and interpersonal skills, with the ability to work effectively with multidisciplinary teamsA valid driver's license and reliable transportationCompensation and Benefits
We offer a competitive salary range of $83,000 to $110,000 per year, based on experience. Additional benefits include:
