A premier long term care company, Concord Care of Hartford, is seeking a skilled Licensed Practical Nurse to join their team. As a LPN, you will play a vital role in providing patient-centered care and promoting a positive team environment.

Responsibilities:
  • Provide high-quality patient care in a long term care setting
  • Collaborate with the healthcare team to develop and implement individualized care plans
  • Conduct thorough assessments and evaluations to identify patient needs
  • Administer medications and treatments as prescribed by physicians
  • Document patient progress and maintain accurate records
Requirements:
  • Graduate of an accredited school of Licensed Practical Nursing
  • Previous long term care experience required
  • Supervisory experience and organizational skills in a long term care setting preferred
  • Must meet health assessment requirements, including two stage Mantoux skin test
  • Must be listed in good standing as confirmed on the Ohio State Department of Professional Credentialing
  • Must be able to communicate in English, both verbally and in writing
