Compassionate Patient Care Coordinator

As a Registered Nurse at Prisma Health, you will have the opportunity to make a meaningful impact in the lives of our patients. We are seeking a skilled and compassionate nurse to join our team on 10 East Medical Telemetry/Nursing.
About the RoleThis is an acute care setting that requires staff to work a significant amount of shift, weekend, holiday, and on-call hours. Our unit supports overflow for Neuroscience patients and offers telemetry if needed. You will work closely with physician groups from family and internal medicine to provide high-quality patient care.
Key Responsibilities- Assessments: Collect data and complete nursing assessments using appropriate area and/or age-specific standards/policies and tools. Complete necessary forms and identify subtle or actual changes in patients' status and patient response to intervention.
- Planning/Outcomes: Develop individual plans of care based on the age and assessed needs of the patient. Collaborate with the patient and/or family, physician, and other healthcare team members to update the plan as needed. Identify realistic and measurable goals and outcomes to be attained prior to discharge.
- Implementation: Provide direct care to assigned patients consistent with the established nursing plan. Delegate nursing tasks as appropriate and ensure completion of assigned tasks. Monitor quality of care delivered on a continual basis and provide care based on patient and unit priorities.
We are looking for a registered nurse with at least one year of experience in a similar role. A Bachelor's Degree in Nursing is preferred, but an Associate's Degree will be considered with relevant experience. Licenses to practice as a Registered Nurse in South Carolina are required. In lieu of one year of registered nurse experience, we will accept successful completion of the Prisma Health Nurse Residency Program or comparable residency from another organization.
