About the Role

This is a dynamic opportunity for a dedicated RN to provide comprehensive care to patients with acute stroke needs. As a key member of our interdisciplinary team, you will work closely with physicians, therapists, and other healthcare professionals to develop and implement individualized care plans that prioritize patient-centered outcomes.

Responsibilities
  • Conduct thorough assessments of patients' physical and emotional needs, identifying opportunities for intervention and collaboration with other healthcare providers.
  • Develop and implement evidence-based care plans that address the unique needs of each patient, incorporating principles of rehabilitation and prevention.
  • Foster effective communication among healthcare teams, ensuring seamless coordination of care and minimizing delays or misunderstandings.
  • Provide education and support to patients and their families, empowering them to participate actively in the recovery process.
  • Collaborate with multidisciplinary teams to optimize patient outcomes, including regular monitoring and evaluation of care plans.
Requirements
  • Bachelor's degree in Nursing required; Master's degree preferred.
  • Current Massachusetts RN license.
  • Proven experience in a hospital setting, preferably in a specialty area such as stroke or neurology.
  • Excellent communication and interpersonal skills, with ability to build strong relationships with patients, families, and colleagues.
  • Proficiency in electronic medical records and Microsoft Office applications.
