At Sea Mar Community Health Centers, we are seeking a highly motivated and compassionate Patient Health Advocate to join our team.
Job SummaryThis full-time position is an excellent opportunity for individuals who are passionate about healthcare and community service. As a Patient Health Advocate, you will play a vital role in ensuring that patients receive high-quality care and navigate the healthcare system effectively.
Responsibilities- Schedule appointments and provide support to patients in accessing necessary healthcare services
- Initiate communication with patients and facilitate referrals to address their comprehensive needs
- Participate in mandatory trainings, service projects, and team-building activities throughout your 10-11 month term of service
- Create informational materials and resources to promote healthy lifestyles and provide access to community services
- Develop a community resource guide to share with patients and colleagues
- Review electronic health records and identify patients requiring important health screenings and services
- Prepare mailouts with relevant materials and patient information
- Collaborate with medical clinic staff, quality improvement teams, and other departments to ensure patients receive preventative care and timely reminders
- Assist in rolling out additional preventive health screening and recall efforts
- Contribute to an AmeriCorps site health project by planning and conducting outreach activities, participating in community events, and promoting public awareness of health coverage enrollment opportunities
- Be at least 17 years old or an out-of-school youth 16 years old at the commencement of service
- Hold a high school diploma or GED, or agree to obtain prior to using the education award; or obtain a waiver under special circumstances
- Be a citizen, national, or lawful permanent resident of the United States
- Have excellent verbal and written communication skills, including ability to compile, interpret, and utilize information
- Be able to work with minimal supervision, use good judgment, meet deadlines, and prioritize workloads
- Maintain confidentiality, professionalism, and appropriate boundaries in all interactions
- Be computer literate and possess good mathematical skills; experience in Windows Office programs preferred
- Reliable transportation required
