About This Role

We are seeking a skilled and compassionate Home Care Manager to lead our residential operations team. As a key member of our management team, you will be responsible for ensuring the highest quality care and support for individuals with disabilities in our residential settings.

Job Summary

The Home Care Manager will oversee the daily operations of our residential homes, including staff supervision, resident care planning, and program development. You will work closely with our clinical and nursing teams to ensure seamless care coordination and excellent outcomes for our residents.

Responsibilities
  • Supervise and manage a team of direct support professionals to provide high-quality care and support to residents
  • Develop and implement individualized care plans that meet the unique needs and goals of each resident
  • Collaborate with clinical and nursing teams to ensure comprehensive care coordination and excellent health outcomes
  • Manage program budgets, staffing, and resources to ensure efficient and effective operations
  • Maintain accurate records and documentation, including resident files, staff evaluations, and program reports
  • Ensure compliance with regulatory requirements, industry standards, and Bridgewell policies and procedures
Requirements
  • Bachelor's degree in a related field (social work, healthcare administration, or similar)
  • At least 2 years of experience in home care management, residential operations, or a related field
  • Strong leadership and supervisory skills, with experience managing direct support professionals
  • Excellent communication and interpersonal skills, with ability to build strong relationships with residents, families, and staff
  • Ability to work independently and as part of a team, with strong problem-solving and conflict resolution skills
Preferred Qualifications
  • Master's degree in a related field (healthcare administration, social work, or similar)
  • Experience working with individuals with disabilities, mental health conditions, or other complex needs
  • Certification in a related field (e.g., CMC, CDMS, or similar)
