As a PRN Case Manager, you will play a vital role in delivering high-quality, client-centered care by overseeing service plans, conducting client assessments, performing supervisory visits, and maintaining detailed documentation to meet Georgia state home care regulations.

Key Responsibilities
  • Conduct Initial Assessments: Perform thorough client assessments, gathering comprehensive information about medical history, current needs, and goals to develop customized care plans.
  • Oversee Service Plans: Regularly review and update client service plans to align with their evolving needs, coordinating with the care team to ensure consistent, client-centered care.
  • Supervisory Visits: Conduct supervisory visits to client homes to evaluate the quality of care, ensure compliance with the care plan, and provide guidance as needed.
  • Documentation and Compliance: Maintain accurate and timely documentation for all client interactions, care plans, and reports, adhering to Georgia state regulations for home care.
  • Communication and Collaboration: Communicate effectively with clients, families, caregivers, and the care team to foster a supportive, coordinated approach to care.
  • Quality Assurance: Participate in quality assurance activities to uphold high standards of care and continuous improvement.

This is a flexible PRN position that offers competitive pay ($45 per hour) and opportunities to adjust your schedule based on your availability.
