We are a locally owned and operated agency, dedicated to improving the health and independence of our patients. Our highly trained clinicians use Oasis to admit, transfer, recertify, and discharge home health patients under physician orders.

Job Summary

We are seeking a Home Health Registered Nurse (RN) to join our team. As a key member of our clinical staff, you will coordinate and supervise LPNs and HHA's in the delivery of patient care. Your expertise will be essential in maintaining compliance with agency policy and procedures, as well as Medicare and Medicaid regulations.

Responsibilities:
  • Initial Patient Visits: Perform initial home care patient visits and re-evaluate patient needs and progress on a regular basis.
  • Plan of Care: Initiate the plan of care under physician orders, performing admission, transfer, recertification, resumption of care, and discharge Oasis for home care patients.
  • Symptom Assessment: Observe, assess, and document symptoms, monitoring reactions and patient progress.
  • Patient Education: Educate patients and caregivers on disease processes, medications, plan of care, and individualized treatment plans.
  • Technique Training: Educate patients and caregivers on techniques for in-home care.
  • Service Coordination: Coordinate patient services, supervising LPNs and HHA's.
  • Physician Notification: Notify the physician and other personnel (DON, PT, Case Manager) of any changes in the patient's condition.
  • Discharge Planning: Discharge the patient from skilled nursing services when discharge criteria have been met.
  • Clinical Records: Update clinical records according to policy and procedures.
Requirements

To be considered for this role, you must meet the following requirements:

  • Nursing Degree: Graduate of an approved school of professional nursing.
  • Licensure: Currently licensed in this Agency's practicing State.
  • Experience: Minimum of one year of nursing experience as a RN.
  • Communication Skills: Work positively and favorably with patients, families, and staff.
Compensation and Benefits

The estimated salary for this position is $85,000 - $110,000 per year, depending on experience. In addition to competitive pay, we offer a range of benefits, including medical, dental, and vision insurance, 401(k) matching, and paid time off.
