Gentiva Hospice, a leading provider of hospice and palliative care, is seeking a compassionate Registered Nurse to join our team as a Nurse Care Specialist for Hospice Patients.

About the Role

As a Nurse Care Specialist, you will play a critical role in providing high-quality patient care and support to patients and their families after normal business hours. Your primary responsibility will be to manage an interdisciplinary group consisting of Licensed Practical/Vocational Nurses, Certified Nursing Assistants, and support staff to enhance the quality of hospice care.

Responsibilities
  • Provide direct patient care services to hospice patients and supportive care to the patient's family on an on-call basis.
  • Evaluate patient/family needs and assist in the preparation and execution of the plan of care for each patient.
  • Maintain all assigned patient clinical records.
  • Participate in company quality assessment and performance improvement programs.
Requirements
  • Compassionate and empathetic approach towards patients and their families.
  • A current RN license (in the state of requested employment or ability to obtain).
  • 1+ years of nursing experience in hospice, home health, or oncology preferred.
