About the Role

This is an exciting role for Nurse Practitioners who are passionate about delivering high-quality patient care and making a positive impact on the healthcare system. In this position, you will work closely with a multidisciplinary team to provide comprehensive assessments, manage acute medical situations, and develop personalized care plans for patients with complex needs.

The ideal candidate will have a strong background in gerontology and experience working with patients with chronic illnesses. You should be proficient in electronic medical records and comfortable communicating with patients, caregivers, and healthcare providers.

Responsibilities
  • Conduct comprehensive assessments and develop individualized care plans
  • Manage acute medical situations and provide timely interventions
  • Collaborate with the interdisciplinary care team to ensure seamless coordination of care
  • Provide advanced illness and end-of-life discussions to patients and their families
