As a Hospice Registered Nurse/RN Case Manager, you will be at the forefront of this mission, working closely with our interdisciplinary team to provide holistic care that addresses the physical, emotional, and spiritual needs of our patients.

About the Role

This is a full-time position that requires strong clinical skills, empathy, and excellent communication abilities. You will be responsible for:

  • Care Planning and Coordination: Develop and implement personalized care plans that address the unique needs of each patient.
  • Patient Advocacy: Act as a liaison between patients, families, physicians, and other healthcare professionals to ensure seamless coordination of care.
  • Clinical Expertise: Provide high-quality nursing care, including wound management, pain control, and symptom management.
  • Education and Support: Educate patients and families on disease management, self-care, and coping strategies.

Requirements and Qualifications

To succeed in this role, you will need:

  • RN License: Current RN license in the state where you wish to work (or eligibility to obtain).
  • Nursing Experience: At least one year of experience in hospice, home health, ICU, geriatric, or oncology settings.
  • Additional Skills: Strong communication, problem-solving, and time management skills, as well as the ability to work independently and collaboratively within a team.
