About the Role

We are seeking an experienced Healthcare Liaison Specialist to join our dynamic team. In this role, you will be responsible for delivering high-quality patient care, promoting timely access to healthcare services, and facilitating effective communication between patients, providers, and caregivers.

Key Responsibilities:

  • Conduct direct outreach to patients via phone, in-person meetings, mailings, and other strategies to educate them about Upward Health's services and support their healthcare needs.
  • Collaborate with patients, families, and healthcare providers to develop personalized care plans that address physical, behavioral, and social health concerns.
  • Capture accurate and timely documentation in our electronic medical record system to ensure seamless coordination of care.
  • Facilitate appointments between patients and care team members as needed, utilizing strong organizational skills to manage multiple priorities and deadlines.
  • Monitor patients' health status, detect potential issues, and provide proactive interventions to prevent hospitalizations and emergency room visits.
  • Analyze patient data to identify trends and areas for improvement, informing care plan adjustments and service enhancements.
  • Briefly explain complex medical information to patients and family members, empowering them to make informed decisions about their care.
  • Collect and document patient feedback, using it to improve patient satisfaction and overall quality of care.
  • Administer basic health care or medical treatments as directed by Upward Health's care providers, ensuring safe and effective delivery of care.
  • Maintain confidentiality and professionalism when interacting with patients, families, and colleagues, adhering to strict HIPAA guidelines and company policies.

Requirements:

  • A minimum of 3 years of experience in a similar role or relevant field (e.g., nursing, case management, healthcare administration).
  • High school diploma or equivalent required; associate's or bachelor's degree preferred.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Ability to maintain up-to-date knowledge of relevant healthcare regulations, laws, and best practices.
  • Strong analytical and problem-solving skills, with the ability to think critically and make sound judgments.
  • Familiarity with electronic medical records systems and ability to learn new software quickly.
  • Valid driver's license and reliable transportation for home visits and community outreach.
  • Flexibility to work varying schedules, including evenings, weekends, and holidays, as needed.
