Job Summary and Qualifications

We are seeking an experienced Registered Nurse with a passion for case management to join our team. In this role, you will be responsible for coordinating patient care, managing lengths of stay, and ensuring appropriate resource utilization. You will work closely with our multidisciplinary team to develop safe and effective discharge plans, promoting quality outcomes and patient throughput.

Your responsibilities will include:

  • Providing case management services for inpatient and observation patients
  • Identifying patients at risk for adverse outcomes during transitions of care
  • Performing comprehensive assessments of patient needs and resources
  • Reassessing patient clinical conditions as indicated
  • Developing strategies to mitigate readmission risks and educate patients on accessing healthcare resources
Requirements

To be successful in this role, you will need:

  • 2+ years' experience in case management or 3+ years' experience in clinical nursing
  • InterQual experience preferred
  • Associate Degree in Nursing or Nursing Diploma
  • Bachelor's Degree in Nursing preferred
  • Current licensure as a Registered Nurse in the state(s) of practice according to law and regulation
  • Certification in Case Management preferred

We offer a competitive salary range of $95,000 - $115,000 per year, depending on experience.
