Job Summary

We are seeking a highly skilled and compassionate Clinical Care Coordinator to join our team at Project 180, a leading provider of innovative forensic treatment programs. The successful candidate will be responsible for providing high-quality clinical care to clients living in transitional housing sites, working collaboratively with a multidisciplinary treatment team to ensure seamless service delivery.

Project 180 is a dynamic organization that turns lives around through innovative, wraparound services. Our programs span criminal justice intercepts, including pre-arrest interventions, court-based diversions, jail in-reach, and intensive post-release services. We operate in partnership with County agencies, including the Department of Mental Health (DMH), the Department of Health Services (DHS), and the Justice, Care and Opportunities Department (JCOD).

Key Responsibilities
  • Direct Clinical Care: Provide comprehensive clinical care to clients, ensuring high-quality service delivery and outcomes. This includes actively engaging clients in services, completing medical assessments, linking clients to needed physical health services, and managing onsite medication administration practices.
  • Care Coordination: Collaborate with the treatment team to coordinate client care, including supporting triage of client medical issues, escalating access to medical care as necessary, and contributing to planning and execution of clinical interventions.
  • Administrative Duties: Maintain accurate records, including medication administration data entry, billing records, and health insurance eligibility checks. Ensure the medication room and storage meet program guidelines.
Requirements
  • Licensure: Active Vocational Nurse license in the state of California required.
  • Experience: Experience working with culturally diverse populations and mental health or forensic population experience preferred.
  • Communication Skills: Excellent communication and interpersonal skills, with the ability to build rapport with clients and colleagues alike.
  • Physical Requirements: Ability to move about the project-based housing site, which may involve walking, standing, and climbing stairs, or using a mobility device.
Compensation and Benefits

The estimated salary for this position is $34.50-$38.00 per hour, based on experience and qualifications. We offer a comprehensive benefits package, including health insurance, retirement plan, and paid time off.
