Job Summary:

As a Per Diem Speech Language Pathologist with CenterWell Home Health, you will have the opportunity to work with patients in their homes or facilities, providing evaluation, direction, and therapy services to help them achieve their communication goals.

Key Responsibilities:

Evaluate and direct speech/language pathology services to patients in their homes or facilities
Participate in creating and reviewing the Plan of Treatment and Plan of Care
Administer and interpret diagnostic tests and therapeutic treatments, including audio logic screening
Observe, record, and report changes in patient's condition and response to treatment
Provide training to patients and families in alternative communication systems when necessary
Maintain documentation as required and submit timely reports
Participate in care coordination activities and discharge planning

Qualifications:

Certification of Clinical Competence in Speech Language Pathology
Minimum six months experience as a speech therapist/speech language pathologist
Home Health experience a plus
Valid license and CPR certification
Good organizational and communication skills
Valid driver's license, auto insurance, and reliable transportation required
