Nurse Practitioner

We are seeking a highly skilled Registered Nurse to join our team as a Patient Care Coordinator in our Call Center. As a key member of our healthcare team, you will be responsible for providing exceptional patient care and support through phone triage and coordination of patient services.
Key Responsibilities- Provide clinical guidance and support to patients and families through phone triage and coordination of patient services
- Collaborate with the Lead Staff Nurse to evaluate and improve call center operations and patient care outcomes
- Develop and implement protocols and procedures to ensure efficient and effective use of human and material resources
- Participate in data collection and analysis to inform evidence-based practice and quality improvement initiatives
- Provide education and support to patients and families on diagnostic procedures, medications, nutrition, and maintenance of health and wellness
- Graduate of an accredited/approved school of nursing: Bachelor's Degree Baccalaureate degree in nursing (BSN) from an accredited school of nursing
- Minimum 1 year professional nursing experience
- Critical care or clinic experience with phone triage preferred
- Knowledge of professional nursing theory, regulations, and practices to give and evaluate patient care
- Ability to maintain effective working relationships with patients, medical staff, and the public
- Ability to react calmly and competently in emergency situations
- Ability to communicate clearly and effectively in person, in writing, or by phone
- Knowledge of clinic policies and procedures
- Experience with evidence-based practice and research
