**About the Role**

We are seeking a highly motivated and compassionate Licensed Practical Nurse (LPN) to join our team at Caroline Nursing and Rehab. As a member of our nursing staff, you will play a vital role in providing exceptional care to our residents.

**Responsibilities:**

  • Provide high-quality nursing care to residents to maintain or attain their highest practical level of functioning.
  • Assess residents for any decline in condition, signs of change, weight loss, decline in ambulation, changes in ADLs, development of skin breakdown, etc.
  • Notify appropriate staff per facility protocol of the need for evaluation and inform staff on any changes in assessment when appropriate.
  • Administer medications and treatments to residents, including monitoring intravenous fluids and placing orders for medication.
  • Confer with residents and families on any issues relating to care and services.
  • Make referrals to other departments when necessary.
  • Complete employee and resident incident reports and follow-up investigation forms.
  • Maintain accountability for controlled substances by reviewing inventory medications with another licensed nurse and notifying supervisors of any discrepancies.
  • Supervise GNAs and unit clerks regarding work procedures, expediting workflow, evaluations, counseling, and discipline.

**Requirements:**

  • Current LPN licensure required.
  • Dentition, MD location preferred.
