Social Services Professional

Job Summary: We are seeking a skilled Social Services Professional to join our team at Luther Manor, a Christian-based Life Plan Community. The ideal candidate will have a passion for providing medically related social services and a commitment to enhancing the psychosocial, mental, and emotional needs of our residents.
Key Responsibilities:
- Assess individual needs and goals to maintain or enhance the psychosocial, mental, and emotional needs of residents
- Act as a liaison between residents, families, and the interdisciplinary team
- Document all resident care plan information while following State and Federal codes
- Assess residents at admission, upon change in condition, and/or annually to identify and seek ways to support residents' individual needs
- Participate in a weekend rotation of on-call for a social service department to address resident and family concerns, timely admission and discharge concerns, and planning
Requirements:
- Bachelor's Degree in Social Work or other related behavioral science
- Social Work certificate for the state of Wisconsin
- Three to five years experience in one of the following environments: assisted living, long-term care, or skilled nursing
- Physical mobility to spend 50% of the workday moving throughout the facility to visit residents under caseload
- Ability to document resident care plans concisely and clearly
- Ability to be patient, sensitive, and receptive to both the residents and their family's needs
- Must be available for on-call for weekend concerns or resident admission on a rotation basis
- Attend in-services/classes concerning the social workers field and maintain social work certification
